The focus of targeted outreach is to identify and contact individuals who are considered at high risk for contracting HIV, determining their HIV status, assessing need and referring them to medical services. Funding for the Targeted Outreach Program is provided through a grant from the United States Department of Health and Human Services, HIV/AIDS Bureau, Ryan White HIV/AIDS Treatment Modernization Act, Part A.

The Targeted Outreach Program seeks to make contact with HIV positive individuals who are not currently receiving primary medical care services. Our outreach efforts include supporting the individual in getting connected to medical services and making referrals to other services needed. If you or someone you know is HIV positive and not currently receiving medical services, please contact the Outreach Team (602) 307-5330 extension 2337. Our outreach services are free and confidential.
